Do you take my insurance?
Amplify Physical Therapy & Performance is considered an “out-of-network” provider. This allows us to treat the individual person in front of us and not the diagnostic code and provide you the appropriate number of sessions without in-network insurance cutting you off. We do provide superbills for people with out-of-network benefits, and we do accept HSA/FSA.
How do I know if I have out-of-network coverage?

What is a superbill?
A superbill is a medical receipt that we will provide you when you pay upfront with us after your session. You can use this receipt as a way to get reimbursed directly through your insurance depending on your benefits.
How long do your sessions typically last?
Your evaluation and follow-up sessions will all be 1-hour long and will be one-on-one with your performance physical therapist.
Do I need a physician's referral?
No! New Jersey is a direct access state, which means that you do not need to initially see a physician or orthopedist for a prescription or referral before coming to physical therapy. Our Doctor’s advanced training allows them to diagnose and treat based on impairments but also the knowledge to refer to a licensed medical practitioner if the condition lies outside their scope of physical therapy practice.
What should I wear to my physical therapy sessions?
Please wear athletic attire so you can move freely and your Doctor of Physical Therapy can access the area of pain or injury. We are a movement-based company, so you can expect to be moving during each session.
How do you do physical therapy differently compared to other physical therapy clinics?
Most physical therapy clinics are built around insurance limitations, rushed appointments, and high patient volume. That often means short sessions, generic exercises, and being passed between multiple providers.

What is your cancellation/rescheduling policy?
If you cancel or reschedule, we require a 24 hours advance notice. If you cancel or reschedule within 24 hours, you will be charged for the session.
